What is the Swing Bed Program?
Swing Bed is an alternative service when a patient no longer needs acute care, yet they may not feel strong enough to return home.
Our Swing Bed post-acute rehabilitation service is designed to provide patients with individualized, in-hospital care and physical rehabilitation to help them reach an optimal level of functioning.
Our team is dedicated to working to regain a patient’s strength, functionality, balance, and range of motion patients need to care for themselves with confidence.
What Patients are Qualified?
Qualified patients who have been admitted to Hills & Dales General Hospital for acute care can transfer to our Swing Bed Program for their rehabilitation phase of recovery and other skilled services without ever leaving the hospital.
You’ll find that Medicare and some other select insurance companies cover post-acute rehab services.

Certain insurance companies require an authorization. Straight Medicare does not require authorization, but patients need to meet certain criteria.
Our Swing Bed Coordinator will assist with determining whether your insurance covers post-acute services.
Some insurances require a three-day acute care stay within 30 days unless there is a waiver in effect or authorization needed from insurance.

What Will the Program Provide For You?
Patients receive skilled rehabilitation, evaluation and treatment services five to seven days per week, or as prescribed. The multi-disciplinary team providing these services includes:
The Swing Bed Program is designed to increase your independence and help prepare you for returning home. Our expert care team will work with you to create an individualized, multi-disciplinary treatment plan that outlines recovery goals focused on restoring you to your normal level of activity. The program is focused on independence, so as a Swing Bed Patient, you will be encouraged to do as much as you can for yourself.
